I, the participant hereby give my consent to my participation in curling and pickleball at the Barrie Curling Club. I assume all risks and hazards incidental to such participation, including transportation to and from the activity and including the parking lot and any portion of the Barrie Curling Club. I waive, release, absolve indemnity and agree to hold harmless, the Barrie Curling Club, the organizers and its operators, participants, officials, organizers and Corporation owners of the premises. Also, persons transporting me to or from activities for any claim arising out of an injury to me.
I understand and agree that there is a risk of injury when participating in any physical activity including curling and pickleball. I release the Barrie Curling Club from all liability arising from personal injury or death and/or damage or theft of my personal property.
I agree to the release of my contact information including email address to the Club Manager and League convenors to keep informed of Barrie Curling Club affairs. I consent to the use of my e-mail address for use in communicating information regarding club events.
I understand and acknowledge that I may be subject to digital recording (including photography) in public areas of the Barrie Curling Club.
Additional forms may be required prior to curling (eg. Rowan's Law forms for curlers under the age of 26). You will be emailed a link to the appropriate forms if this applies.
